The Cardiothoracic Centre - Liverpool is a tertiary referral
centre for the investigation and treatment of adult cardiac
and thoracic disorders. The centre serves a population of
2.8million.
There are four Catheter Laboratories and seven theatres. In
2002/3 the workload of the Unit included 1250 Coronary Interventional
procedures plus 30 PTMCs, in excess of 4,500 invasive cardiac
investigations, 1000 permanent pacing procedures, 1500 CABG
procedures and 250 other cardiac operations.

Clinical Experience
I have since appointment undertaken a heavy clinical workload
in my major area of interest i.e. Interventional Cardiology
with a total experience in excess of 4,500 cases. Year on
year I have performed the largest number of coronary interventions
in an increasing variety of patient subgroups. In addition
I have until recently been heavily involved in the clinical
development of Echocardiography.
Developments in Interventional Cardiology (ongoing); the introduction
of new techniques to CTC including lead use of intracoronary
ultrasound, first/lead user in UK of Transluminal Extraction
Atherectomy and Barath Cutting Balloon Angioplasty, development
of cost saving coronary stent regimes. Collection of Audit
Data on Interventional Cardiology. Recently I have led the
development of groin sealing post PTCA and the radial PCI
programme with a view to early mobilisation and discharge.
Director of Echocardiography 1991 - 2000: overseeing
of training in Echocardiography; the development of new technology
including multiplane TOE; the recent setting up of the a stress
echocardiography service including facilities for contrast
echo; the development and reporting of a GP based open access
echocardiography service.

Medico-Legal Experience
I have extensive experience of performing medico-legal examinations
and writing reports for solicitors, crown court, coroner's
court and others. I am registered with a number of medico-legal
agencies and have attended a variety of courses in medico-legal
work. I have made crown court appearances on various occasions
for both prosecution and defence.

Teaching/Training
I have been involved in both undergraduate and postgraduate
teaching for many years and regard this as a major commitment
both in my role as postgraduate tutor and in the development
of the Calman training program.

I arrange and chair the weekly cardiology seminars which are
principally for the Junior Medical staff but are attended
by a wide range of the CTC staff. I participate in the regular
local MRCP Teaching and the regional MRCP Roadshow. I teach
nurses studying for MSc in nursing who are regularly attached
to my firm. Medical Student teaching during term time both
at CTC and during block teaching at the University. I teach
junior doctors and technicians on echocardiography.

Research and Audit
Member of Primary Care Audit Group 1995-1998: mainly in the
development of GP based protocols of management of Heart Failure
in the Community
Collection of Interventional Data on behalf of BCIS 1989-present
My main areas if research have been in intervention and coronary
artery disease and also in the evaluation of LV diastolic
function. I have collaborated closely with the research Lab
Staff in establishing various assays in particular in evaluation
of platelet function and markers of restenosis. The following
paragraph expands on this and I have listed the three publications
I feel are most significant.
Assessment of diastolic left ventricular function including
correlation of invasive and non-invasive measures and the
effect of various stressors most recently isometric handgrip.
The triggering of anginal events particularly in unstable
angina and the role of platelet activity and inhalation of
cold air. I have been involved in National Trials in coronary
intervention including supervising the SOS Trial, the BRIT
Study, PRESTO, CARISA and CHOICE 2000, also presently CARDIA
and ACUUTY. I have also been an active participant in the
RITA 2 Trial and COVERT Study as well as a number of trials
of therapeutic agents in angina and heart failure. I have
three further major projects in development in conjunction
with my junior staff involving the scientific basis of restenosis
in various clinical subgroups undergoing intervention. I have
funded three and supervised four research fellows (1991-present)
all of whom have produced work suitable for submission for
MD theses. I have been a member of the R & D Strategy
Group and the Research Grants Committee at CTC and was involved
in the establishment of the two CTC Senior Lecturer posts
and the development of links with the University

reviews and peer reviewed original research.
